A 40-year-old man has a chronic cough with fever for several months. The chest radiograph reveals a diffuse reticulonodular pattern. Microscopically on transbronchial biopsy there are focal areas of inflammation containing epithelioid cell granuloma, Langhans giant cells, and lymphocytes.
