A 52 year old male executive is seen in the casualty with hypotension, vomiting bright red blood at home and in the hospital. There is no previous suggestive history. The estimated blood loss is around 2 liters. The diagnosis is


a) esophageal varices  
b) gastritis   
c) duodenal ulcer 
d) Mallory-Weiss tear
